Role: Mental Health Social Prescriber

Location: Regis Coastal PCN- Working in one GP Practice

Salary: £27,787.50 per annum (Pro-Rata)

Hours: Part-Time – 15.00 hours per week

An exciting opportunity has arisen within our community based mental health services in Bognor Regis West Sussex.

Making it happen – Your Role:

You will be joining an established team which supports people with mental health needs.

The role of The Mental Health Social Prescriber is really varied and extremely rewarding, the job role will involve:

  • Establishing and maintaining a regular presence at primary care GP practices.
  • Working in partnership with key practice staff, clinicians and community services.
  • Providing timely support to people identified as requiring assistance.
  • Using established processes to assess the requirements of people in need.
  • Supporting people to identify their needs and goals.
  • Developing support and recovery plans.
  • Signposting people to community resources and services.
  • Maintaining accurate and up to date electronic records.
  • Ensuring compliance with multi-agency policies in general but specifically safeguarding adults and children.
  • Adherence to all health and safety regulations within GP practices and United Response.
  • Undertake all mandatory training and training identified as necessary for the role.

About You:

To be an excellent Mental Health Social Prescriber you will have:

  • Evidence of previous training or learning (formal or informal) which relates to supporting adults with mental health needs.
  • Excellent literacy, IT and verbal communication skills.
  • Experience of supporting people with Mental Health needs
  • Ability to manage your own caseload.
  • A full clean driving licence and use of a car is desirable
